Add Additional Notes to the DBT Diary Card
- Log into the mindLAMP app or dashboard.
- Navigate to the Assess tab.
- Click on DBT Diary Card.
- Select "Start Survey."
- Click "Continue" after seeing your life worth living goal.
- Complete your target behaviors and select "Next."
- Complete the emotion page and select "Next."
- Next, you will be prompted to answer if you have used any skills. Select Yes or No.
- If you select yes, complete the skills sections and select "Next."
- You may include any details about your day in the text box. Once you are finished, click "Submit."
